This is a list of commonly requested forms that you can download and print.

American Water Forms
This is the form you will use to select the payment method you want in the event that you are out on Workmen's Compensation and you are employed by any of the Pennsylvania American Water Districts. Depending on the time of year, your sick leave accrual, and length of service, these options offer varying solutions for your situation. >click here<

The form on the next page will enable you to opt out of the American Water iPay process. You will then be provided a paper pay stub. Be aware, this will not get you out of the direct deposit process. Most of our American Water contracts require direct deposit once enrolled. Check your local contract to see if it binds you to iPay. As of 2017, none of our contracts require you to use the iPay system. >click here<

Some expenses that you incur at work are reimbursable. If you have an expense and need to fill out a form to get reimbursed, this form will help. Just download to your computer and fill out the required information. In most cases, you will provide this completed form to Human Resources. >click here<

All Employees/Members
The Pennsylvania Workmen's Compensation handbook provided by Jim Welker at Jubelirer, Pass & Intrieri is a good resource. It is always a good idea to discuss your situation with a local Union representative. If you believe your case is going to involve twists and turns, Local 537 recommends Jim. He has helped numerous Local 537 members. Make sure you tell him you are a member in good standing in Local 537. >click here<
